What Causes Severe Abdominal Pain When Diagnosed With Acute Prostatitis?

Hi, To start with, I visited ER due to severe abdominal pain in the month of Oct 2015. In the same month, CT scan abdomen and pelvis, Ultrasound, erect abdomen radiograph was done and all was normal. Urine had shown 0-2 RBC cells/hpf and since then it has been consistent till now. I even had few protein in my urine but it eventually stopped. Finally in the same month I was diagnosed finally with acute prostatitis so started the antiobiotic dose. In the month of Dec 2015, I had a cystoscopy and MRI abdomen and pelvis and it was all normal except that I had mild bladder wall thickening bladder neck obstruction which doctors suspected would have occured due to prostatitis. Just to mention I also had upper GI endoscopy and colonoscopy done in the month of Dec 2015 which was also normal. From January 2016, I started to have persistent pelvic pains which doctors said might have been due to prostaitis and it would go away over time. Nowadays, I am having consistent abdominal pain again though it is not severe and a random urine test revealed protein of 1+ (30 mg/dl). Blood is consistent at 0-2 cells/hpf. I am again having moderate flank pains also, My doctors have referred for a contrast CT scan for abdomen and pelvis. I am a bit worried now. Just to reassure, can I rule out atleast big things like cancers and tumors in my bladder or kidny etc. I already have got mutiple tests done around 8 months back and it was all normal. Or is there a possibility something can grow up in this 8 months time?

General Surgeon 's  Response
Hi,

Noted your history about a case of acute prostatitis diagnosed after investigations and now having pain in abdomen, ruled out other causes like cancer and so on.
The moderate flank pain and other symptoms look to be due to urinary tract infection.
I would advise you the following in such a situation:
Complete the course of an antibiotic for 3 weeks.
Take plenty of oral fluids to have good urine output and this helps in controlling UTI.
If there is an abscess in prostate then you may need manual rupture or it and then only hte antibiotic will be helping.
Culture and sensitivity of urine will give the name of the probable antibiotic of choice and get changed over to the best one your Doctor feels so.
Regular masturbation helps relieve the congestion associated with prostatitis.
Cancer of prostate is not known to occur in young age so no worry about it unless suggested by any investigations.
Hence regular review with your Doctor is a must.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T Chandrakant, ENT & General Surgeon
