Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

It is possible that the pain is due to the diverticulitis which is inflammation/infection of the diverticula in the intestines.

Metronidazole can cause abnormal discomfort but it is not usually associated with severe pain.

Because severe abdominal pain can be due to a surgical cause, it would really be best if he is seen.

I hope this helps.
