Understanding your concern. As per your query you have recurring lump on back with painful upper and lower spine. Well there can be many reasons for symptoms you mention in query like disk herniation , tuberculosis of the spine , spondyloarthropathy , ankylosing spondylitis , scoliosis , hyperlordosis or septic arthritis. I would suggest you to consult orthopedic surgeon for proper examination .Doctor may order CT scan , MRI along with physica examination to check tenderness or consistency . Doctor may recommend surgical excision . For now apply warm compresses , take ibuprofen ot acetaminophen for pain and take proper rest .
Hope your concern has been resolved.
