General Surgeon 's  Response
Hello,

As per my clinical experience, the definite underlying cause of the swelling and redness in ankle is matter of evaluation for the possible differential diagnosis as:
- Recurrent Bursa cyst after excision or incision and drainage
- Chronic cellulitis
- Recurrent bacterial infection of subcutaneous tissue
- Draining sinus of fungal infection or atypical organisms as Nocardia or others

In my clinic, I do recommend thorough clinical examination with peripheral pulsation and may advise for the need of:
- Diabetes profile
- Doppler ultrasound of the lower limb
- Complete hemogram and WBC ount
- X-ray of the right ankle to rule out any bony lesion secondary to this
- MRI as last resort if everything above turns out to be negative only.

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
