Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

We will try to help you in best way possible. First of all it's sad to know what your patient had been going through lately. Based on the history you have shared, it can be vasovagal syncope and cough can trigger it though other possibilities like raised intracranial pressure needs to be ruled out. Cough and other strenuous activities needs to be controlled.

Hope I have answered your query.
Let me know if I can assist you further.
