What Causes Persistent Epigastric Pain, Loss Of Appetite And Whole Body Malaise?

Hi got severe headache most of the time and followed by severe epigastric pain radiating in my back . Loss of appetite and whole body malaise. I am trying to put something as I can t sleep at night due to pain .It started when I woke up early 3 am for my work to travel from the Island going to town and working until 2 pm back to the island and arrived at mostly 1600h ant that is the time I can eat my food. However they gave me boscupan and ranitidine IV and it helps and again pain goes back tried to drink omeprazole 20 mg po at night as prescribed but pain and loss of appetite and whole body malaise with me. Kindly advise please

Hi,

As per your symptoms it could be most probably gastric or duodenal ulcer. Most of it favoured in duodenal ulcer. As you said you used medication but it failed. It indicates non healing ulcer. Most of the time it is due helicobacter pylori infection or chronic use of pain killers.

So from my side meet gastroenterologist get endoscopy done for confirmation of ulcer and h.pylori infection testing. Based on this they will prescribe H. pylori medication for 14 days. Later they re-assess. If it is healing well and good if not it need to further evaluated and treat by surgery. Like vagotomy or reaction of stomach. So kindly get endoscopy done first.
