What Causes Panic Attacks, Breathlessness And Depression?

I and panick attacks when I am in social situations. I also hold my breath unknowingly when I am in social situations, tense up, and have trouble breathing normal. I have had these problems for many months now. I am concerned about my mental health. I have taken many tests/screenings online and they all would tell me I have some symptoms of social anxiety or depression. I have not been diagnosed with anything probably because I always tell the doctor that nothing is wrong. My question is what do you think about my mental health based on the things I described above?
