Hi..
Welcome to HEALTHCARE MAGIC..

I have gone through your query and can understand your concerns..

As per your complain red patch on lower lip with a history of smoking can be due to inflammation caused by continuous irritation caused to the lip skin from heat that is produced from burning Cigarette..
You should completely quit the smoking habit ..
Apply a steroid based ointment over the red patch.
Do cold compresses..
In case if still the redness does not improve then you should consult a Dermatologist and get evaluated and a biopsy can be advised to rule out the cause and treatment can be planned accordingly..

Hope this information helps..

Thanks and regards.
