What Causes Pain In The Ovary With Bloating?

Hello, I was wondering if you could help me asses my current situation. I have had severe pain from my right ovary for about a month now, the pain is ranging from about a 3 (painful, but bearable and very annoying) to about an 8 (almost unbearable). This pain, as well as the uncomfortable bloating, wakes me up at night, it makes normal everyday activities difficult, and work nearly impossible. I haven t been eating well since this pain started, and apparently my urine indicates that my body is in starvation mode. I have lost about 15 pounds in this time. I haven t had much of an appetite, and when I do try to eat the nausea makes it almost impossible. I have basically been living off of ensure. I went to my doctor and she ordered a transvaginal ultrasound, luckily I work in radiology so it was quite easy to get done. As we were in the proses of the ultrasound the tech made an uneasy face and simply said oh”. This didn’t really bother me much; I figured she just realized that I really am in a good amount of pain. I ll share the results of this ultrasound with you at the end of my message. I am 21, sexually active, and haven t recently changed partners. I have a history of thyroid cancer; I was diagnosed and cleared at the age of 14. I don’t know anything about my family history due to adoption. I am really concerned, mostly because the radiologist states in the report that there were no findings. I am very worried that after this nobody will believe how much pain i am in, and therefore I won’t find out what is causing it. Thank you for your time, and assessment. FINDINGS: Transabdominal and transvaginal real-time grayscale sonographic images were obtained. Color Doppler flow analysis is also available for review. PELVIC ULTRASOUND, TRANS ABDOMINAL: The uterus measures 6.5 cm. It is anteverted. The endometrial stripe measures 5.4 millimeters transabdominally. The right ovary measures 2.7 x 4.5 x 3.3 centimeters. It has a normal appearance without evidence of mass lesion. Normal blood flow identified. The left ovary measures 3.2 x 1.9 x 2.5 centimeters. It has a normal appearance without evidence of mass lesion. Normal blood flow identified. PELVIC ULTRASOUND, TRANS VAGINAL: The endometrial stripe measures 6.6 millimeters transvaginally. The right ovary measures 4.4 x 2.3 x 3.7 centimeters.A mildly hemorrhagic dominant follicle is noted within the right ovary measuring up to 2.2 cm in diameter. The right ovary has a normal appearance without evidence of mass lesion. Normal blood flow identified. The left ovary measures 2.0 x 2.1 x 1.9 centimeters.It has a normal appearance without evidence of mass lesion. Normal blood flow identified. Physiologic free fluid is noted within the cul-de-sac measuring approximately 6 cc in volume. IMPRESSION: Unremarkable pelvic ultrasound. No explanation for patient s symptoms..
