What Causes Pain In The Lower Back And Sneezing After A C-section Delivery?

Dear Sir, I am sending this querry with great hope. On 6 Oct’16, my wife has gone through C-section delivering a baby girl. After 15 days of surgery, she started feeling lower backache with very local pain in bending, sneezing & coughing. Initially we thought it is a post cesarean effect and as she was breastfeeding our baby, she took homoeopathic medicines. Afterwards, We consulted orthopedic as there was no relief in pain. In January last, we consulted SGPGI in Neuromedicine Deptt. On 24 Jan’2017, after certain blood investigations and MRI Report they diagnosed it as Pott’s Spine in L4-L5 region. The MRI report says: · Altered T2W1 hyperintense and T1W1 hypointense signal intensities noted involving the L4 and L5 vertebral bodies with paradiscal destruction of L4 vertebral body. Post contrast study reveals heterogeneous involvement. · An irregular peripheral enhancing T2W1 hyperintense and T1W1 hypointense collection is noted in intra-osseous space, prevertebral and paravertebral space extending behind L4 and L5 vertebral level. Small similar collections noted in adjacent psoas muscles. · Anterior epidural space collection noted behind L4/ L5 level causing spinal canal stenosis. · Rest of the vertebrae are normal. They have started the treatment course and suggested to wear LS Belt. The medication is as follows: · RCIN-500: OD · ECONEX-850/300: OD · PZIDE-500 BD · DOLO-650 BD · Syrup DICALCIIPLEX N 1 TS BD Our hometown is 200 km away from Lucknow. Initially when we had gone to Lucknow by car, she was able to walk herself. After 24 hrs of returning, she started feeling severe pain in her back as well in right leg. Actually, in returning, we met with bursting of tyre of car, absorbing jerk on her spine, which I think may be a reason for the same. Right now she is feeling difficulty in moving & walking. Can u please prescribe her what to do next? Does she need some additional medication? It s one month after starting ATT, How much time she will need more to walk alone? Does she need physiotherapy? Thanks in advance

General & Family Physician 's  Response
Hi,

Her problem is due to compression of nerves supplying the affected leg and the recovery would start as the medication and the compression starts releasing. Be in touch with the treating doctor regularly, she may need re-assessment to judge recovery. Few cases need surgery to decompress in certain indications.

Treatment would last for a minimum of 9 months to 1 year. Recovery would start showing till the time.
- She needs rest till recovery as movements would increase the grade of compression & may hamper healing
- Naturally no physiotherapy is advisable till then.
- She also needs high protein diet with vitamin, mineral supplements for good recovery.

Hope I have answered your query. Let me know if I can assist you further.

Thanks.

Regards,
