Hi! when a person comes to me with any kind of knee pain with mobile lump under the knee cap, I usually get following possibilities as per imaging studies, clinical examination and needle biopsy experience:

1. recurrent patellar dislocation 30%

2. synovitis, tendinitis or bursa cyst 30%

3. benign tumors like chondromatosis, osteochondroma etc. 20%

4. loose bodies and forgotten old tears/fracture 10%

5. synovial herniation or vascular tumors 10%

Therefore, he needs rest of the knee involved and please see an orthopedic surgeon for an urgent opinion but please don't worry as this is benign, infective /inflammatory only and never malignant!
