What Causes Low Grade Fever Along With Earache And Cough?

Hi, my daughter is 11 years old and has been sick since June 4th 2017. She first presented with a low grade fever 99.9 and an ear ache, stuffy nose, cough, hurt all over and extremely tired, we took her to Urgent Care, they dismissed it, did nothing and sent her home. Within 2 weeks she became even more tired, her fever went up to 102.3, extremely tired, cough worsened, ear ache, no sore throat, stomach ache, just didn t feel well all over! We took her to the Urgent Care, again. They did a chest xray, blood lab and Urinalysis. Three days later we finally were able to reach the Dr. on results, they said it looks like bronchitis! No UTI, blood showed she had Mononucleosis, end stages of it. Dr. Did put her on Cefdiner 250 mg 11 and 1/2 ml once daily for 10 days. I keep pushing the ibuprofen and expectorant cough syrups. Her cough is getting worse and her fever higher! 3 days after beginning Cefdinir, We took her in to see her Primary Care Physician because we weren t seeing any improvements. They said she had pneumonia, gave her albuterol inhaler! See how she does over the weekend, if her fever was to hit 100.3, or above to call oncall Dr. The next night We ended up taking her to the ER because her temperature spiked to 104.8 axillary! They checked her ears, eyes, breathing, stomach, we felt the Dr. was real thorough! He looked at the previous xray and said that the Radiologist read it as Bronchitis not pneumonia. He said in his opinion it really didn t look that bad! He gave her a breathing treatment with 2 different medicines and a steroid pill that was to last 3 days! Well her fever broke the following day! First time in weeks!! She was still exhausted, coughing even more as the days went on..this was last saturday June 24th... then this Thursday (during the day) she starts with the fever again! Still coughing and it sounds really different, not a bark but different. Hurts all over...extremely tired. I call her PCP and we got her in to see them yesterday. They repeat the chest xray. They call us and say in comparison to the one done on the 19th, the striations are more and one of her lymph nodes in her lung is swollen. The Radiologist recommended a Cat Scan, our Dr. said to give it til the 10th of July, that is our daughters follow up appointment. Her PCP feels that the lymph node is swollen due to her being sick as long as she has been. They feel it is a Virus and it needs to run it s course. I m assuming they will repeat the chest xray at this time and or order the CT Scan if she s still not better. My questions to you are what are the chances this swollen lymph node could be lymphoma, whether Hodgkins Lymphoma or another kind? Also, Is waiting to see how she s feeling in 10 more days the best for my child? If it is Lymphoma, would that necessarily show in the CT Scan? I would like your honest opinion on all of this please. Thank you..Sincerely, Jerilyn P.

General Surgeon 's  Response
Hi,

Possibly seems to be acute otitis media or other issue pertaining to the ear. Primary relief with local antibiotic drops, systemic antibiotics, anti-inflammatory drugs and other supportive care.

Definite treatment with ENT specialist and audiometry, CT scan would be next step to proceed.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel,
