What Causes Intermittent Numbness And Chills In Feet With Pain In Big Toe?

I am an RN doing bedside nursing. Its my feet. they feel almost cold and numb at times. I think sometimes my shoes are tied too tight, but its not that. They also hurt. I try to make sure my shoes have plenty of cush, but the ball of mt lt great toe hurts and it looks like the joint is getting bigger. What could this be?
