Hi
Thanks for posting on HCM
The symptoms you describe even though not so clear may be caused by a local skin infection by a bacterium or fungus, skin irritation, dermatitis or an STI like syphilis or herpes.Just being obese predisposes you to infections around your groin/genital region given the skin folds that are always on this region in obese people.I will suggest you see your GP for primary evaluation. He may request for STI work-ups if need be.If a dermatological problem, he may send you to the Dermatologist. As of now, i can only encourage good genital hygiene till you are examined by a Doctor.Pain medications like ibuprofen may help if pain too serious.
Hope my answer will help you
Best regards
