What Causes Frequent Vaginal Bleeding?

I m having several different concerning symptoms, and not sure what to search for on WebMD. For over a year I ve had my menstrual 2 xs a month, until about 2 months ago - only 7 days between menstruation. For example: I started on Sept 29th (lasted 6 days) longer than my usual 3-4 days, and my first day it was dark more of a brown color then red and heavy. I just started again today, and it s more of a clear with red streaks. When I menstruate off my left side, I always have sever pain, unbearable. I often notice blood clots. I m having night sweats, increased urination, nausea (all day every day), dizziness, weight loss, and when Im not menstruating, I have either clear thick discharge for days, or at times it s a white/clear color. My last pap and ultrasound was Dec 1, 2016. At that time, I was told my pap was abnormal due to having HPV 1st strand, and the lower the strand the lower the risk for cancer. I was also informed that my ultrasound showed a normal uterus size, but my left follicle was swollen. I m concerned because now the bleeding is so frequent. I m not on birth control or any blood thinners. I m only 36 years old, and very active at the gym with weight lifting. I m married, and my husband has had a vasectomy, so no need for birth control. Suggestions? I m not sure what to tell my doctor to look for? If they didn t find anything in Dec that was of concern in the Ultrasound or or Pap not sure what to do next?
