General & Family Physician 's  Response
Hi,

SLE most common manifestation is Kidney problems. Foul smelling urine might stem directly from diseases in the kidney or newly occurring urinary tract infection. Please refer to your nephrologist for further evaluation.

For the fatigue issue, SLE is a rheumatological disease which is commonly associated with fatigue. If you were my patient I would order a blood test to test for anemia.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Ahmad Nazzal, General & Family Physician
