What Causes Extreme Thirst During Night?

I have had extreme thirst during the night for more then 10 years. I have increased my water intake in the last 9 years due to a decrease in GFR. I still have the thirst upon waking, even when I drink a glass or more before I go to bed. My FBS runs around 95 to 100. I am salt and sugar sensitive. I have days where I itch all over and my hair is thinning. My Thyroid tests are within normal range. I have osteoarthritis, sarcoidosis in the lungs and have muscle pain ( usually decreased with 2,00 to 5,000 u of Vit D daily) and fatigue easily even though I try to bike, hike or use elliptical 5 times a week for at least one hour. I also have chronic gastritis and IBS. I ve been on a gluten and dairy free diet for 9 years. What might be causing the thirst, fatigue, itching and feeling like I have a slight flu everyday?

General Surgeon 's  Response
Hello,

As per my clinical experience, the long term condition of extreme thirst with decrease in GFR does indicate possible issue with hormone axis as diabetes Incipidus or kidneys where there will be polyuria with hyponatremia or other symptoms and lab data. You must get fresh evaluation with:
- Blood urea and creatinine with ratio
- Complete urine microscopy with culture
- Serum ADH levels
- Osmolarity of serum and urine
- HBA1c level
- Electrolytes or others as per the clinician need and evaluation

Take care. Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
