Hi,welcome to HCM. Diurnal blood pressure fluctuations with chest discomfort largely point to early features of congestive cardiac failure.I strongly suggest you to undergo an early 2Dimensional echocardiography to document heart failure.A baseline EKG can give clue to elevated heart rate.Exercise testing in the form of tread mill testing can give vital clues to associated coronary ischemia which can precipitate heart failure.Treatment includes antihypertensives and diuretics.Consult your cardiologist for further queries. Thank you
