General & Family Physician 's  Response
Hello dear Greetings
Regarding your experience with ear pain radiating down the feet tinnitus and vertigo
I am suggesting you have developed an otalgia due to one of the following

Paroxysmal positional vertigo
Acute vestibulitis
Menieres disease
Otitis media

In view of this please see another ENT specialist for further clinical assessment and adequate drug prescription

Concerning the twitching sensations in your nerve I will suggest you have developed a clonus though this may be linked to your ear pain though it is a symptom which will be arrested during your ENT appointment

I hope this helps Regards
