General & Family Physician 's  Response
Hello and Welcome to ‘Ask A Doctor’ service.
I have reviewed your query and here is my advice.

Your dizziness and history suggests that you are experiencing the side effects of diltiazem which cause hypotension and bradycardia (slow heart rate).

It warrants for either a decrease in the dose or switching to beta blocker for rate control of a-fib. Please consult your physician or cardiologist in this regard.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. Muhammad Adnan Iqbal
