What Causes Diarrhea And Stomach Upset After Increased Dose On Metformin?

I just recently had my metformin increased to 2 1000 pills a day. I have been having diarrhea and upset stomach. I am also on atorvastatin 10 mg. I am having lots of pain, flu like symptons and just not feeling good! Moody and short tempered. this all started after I up the dose from 500 twice aday. I am on 6 units of insulin per day. could it be either of these meds causing the problem.

Thank you for the query.
I would like to know your age, gender and duration of Diabetes. I would also like to enquire about any other medications which you may be taking apart from Metformin, Insulin and Atorvastatin.
Metformin is known for its gastrointestinal side effects. It can cause nausea and diarrhoea. In some subjects it can also cause lactic acidosis (specially in those with renal impairment). You are taking, 2000 mg (1000mg x 2), of metformin per day, hence it may be that these symptoms are due to Metformin. I would advise you to get a renal function test and liver function test (since you are also on Atorvastatin) done (in case it has not been done). A total leucocyte count may help to rule out an infectious diarrhoea.
Diabetic subjects are known to have GI manifestations like gastroparesis and Short intestinal bacterial overgrowth (the latter can lead to diarrhea), and these must be ruled out.
I would suggest you to visit your physician, so that a physical examination can be performed. If required, your metformin dosage may be reduced. It is also necessary to rule out an infective cause of your diarrhea for which antibiotics may be needed. Though in your case the diarrhoea in all probabilities is due to the increased dose of Metformin. Probiotics may provide some relief in this condition.
Hope this helps you out.
Regards,
Dr. Ayusmati Thakur
