What Causes Brown Vaginal Spotting While On IUD Mirena?

I got the IUD Mirena in October 2013 and had cramps and spotting for the first few months then I didn t have a period. Today I ve been having severe cramps to the point of making me sweat and when I went to take a shower this evening, I noticed brown, spotting blood in my underwear. Should I be concerned?
