What Causes Black And Tarry Stools In An Elderly Person With Muscle Spasms?

my mother is 93 and I am her caretaker daughter. her conditions that concern me the most now are following. She has heart disease and pseudophospholipid syndrome along with patchy moderately erythematous mucosa with minimal blood lose and subsequently treated with iron for the deficiency due to blood loss probably from her blood thinner and blood slicker. she also takes a coating agent, sucralfate, to counteract this. now I cannot tell if she has black and tarry stools due to the iron and grape juice or she is bleeding again in the lower GI tract. how can i tell the difference? so that I can get her to the er. she also lately has severe nightly muscle spasms ( about every 30 seconds) in her legs or leg. could this be due to blood loss?
