What Casues Purple Discoloration On The Feet?

My mother is an ar old woman with two aortic aneurysms, one in the chest area and one near her stomach. She is in stage 4 almost stage 5 kidney failure. She has a fistula in her arm and it has matured and is ready for dialysis. She also has gout and is on a gluten free diet. My question is her feet have turned purple mostly near her toes but on one foot her middle toe is turning black at the tip. Other toes are darker than the purple on the rest of the purple area. Which doctor should I call first? The kidney or vascular?

Blackish and purpish discolouration of tips of toes in background of all medical problems you mentioned indicates she must be getting dry gangrene.
Dry gangrene usually occurs due to reduced blood supply to the peripheral parts as a result of obstruction in blood flow.
You need to go for vascular specialist as early ad possible.Dry gangrene may cause infection or shock or may lead to undergo amputation if not treated timely.
colour doppler of peripheral blood circulation.

Thanks.
