High amount of alcohol use can damage esophagus & stomach directly and by increasing acidity/acid reflux. This damage lead to reflux spasm, pain in chest with cough. Apart from that it damages almost all distant organs of body.

Chronic smoking (irrespective of number of cigarette smoked per day) can lead to cardiac problem, damage to lungs and increase acid secretion. Increase acid production and damage to lung can cause cough and respiratory problem along with chest pain. To know the pathology, investigations like Bronchosopy and upper GI endoscopy are helpful.
