Can be coincidence, can be correlated coincedence (people who chew tobacco do not brush and/or floss as much as those who do not) and,
there can be sand/grit in the tobacco that wears down the teeth over many years. There can be the action of the chewing itself that wears down the teeth. Someone who chews may also grind their teeth, the stimulants in the tobacco will increase this. Then, the chemical and drug actions of the chew will have some consequences (more acidity in the mouth, different saliva flow).
So, a lot of possibilities. Certainly not everyone who chews has this many problems from it. I know many people who chew and they don't get a lot of sinus problems so it certainly isn't guaranteed the way discoloration of teeth from chewing IS guaranteed, but there are many ways it could be connected in particular people.
