Hello, and I hope I can help you today.

There is no way to know in advance if Clomid (Clomiphene Citrate) will work to make you ovulate to achieve pregnancy. Your OB doctor is correct in regulating your cycle first with an oral contraceptive before beginning Clomid- it makes sure you are at the correct time in your cycle at the start and decreases levels of male hormones in your body (one of the hormonal imbalances in PCOS).

The ultrasounds you will get are designed to look at your ovaries and see if a follicle develops and is ovulated. He/she will be able to tell if you ovulated or not on the dose of Clomid you are trying. Even if you ovulate, it is not a guarantee you will conceive, but you should have intercourse every other day starting on day 7 or your cycle until at least day 21 (you can ovulate later than day 14 if you have PCOS and do not normally menstruate. If you don't get a period by day # 35, do a pregnancy test. You will probably repeat the same plan for four cycles or so with increasing doses of Clomid until you ovulate.

In terms of the chance of pregnancy, women with PCOS, even if they do not get periods without birth control pills, respond well to Clomid. I had my own patient who never menstruated without hormone treatments who conceived on her second cycle of Clomid. So I cannot quantify your chance of pregnancy, but I can say if PCOS is your only issue, and you re under 35, your chances are very good for conception. The Diane you took actually increases your chance of successful pregnancy with your Clomid cycle, and I agree with your doctor's plan and regimen.

I hope you are able to achieve a successful pregnancy and that I answered your question.

Best wishes, Dr. Brown
