Suggest Treatment For Testicular Pain When Diagnosed With Chronic Lumbar Pain

Last few days a new pain for me - right testicle mild background ache, sharper pain when first sit down, even sharper when lifting right leg. No leg pain, no swelling, no palpation pain, no urinary problems. On Finasteride for prostate for about 2 years, have good PSI. History - chronic lumbar pain nearly 40 years, some sciatica-like events/symptoms, maybe have "Sacroiliac-Joint-Syndrome". Have had MRIs, X-rays, PT, chiro over the decades, can walk about 250 yards before too much pain. Fall fractured L3 in 2009.

My opinion-
In the given situation-You seem to suffer from-Chronic PID(Prolapse Intrevertebral Disc) with L1 compression causing testicular referred pains.
From 250 yards walking limit-you seem to have Spinal Canal stenosis-mostly after L3 fracture in 2009,which needs second opinion from NeuroSurgeon/ Orthopedic Surgeon.


Do's Suggested-
CT Contrast spine L1-L3 to assess Chronic Lumbar Spinal stenosis-(CLSS)
RFA(Radio Frequency Ablation) / Or ESI(Epidural Steroid Injection) treatment after evaluation with CT, would be advisable to give long remissions from Chronic Lumbar Pain.CRPS(Complex Regional Pain Syndrome) from sympathetic dysrophy post-L3 Fall fracture,needs to be kept in Mind-while treating -if pain does not respond to above treatments.
Your 40 yrs history increases this possibility being missed out and needs treatment on that direction
Your prostate is normal and is not reason for testicular pain,where L1-neurogenic pain seems to be most probable possibility in
your case.
USG scan with Doppler Study of the affected testes to rule out Varicocele with Hernia, for your age- needs to be ruled out in your case.
Consults with General Surgeon/Urologist/ Orthopedic/ or NeuroSurgeon team would be needed to treat the problem.

Other causes need to be ruled out after Consults with Specialist Urologist and Ortho-Surgeon would be needed to fix your testes pains with CLSS(Chronic Lumbar Spinal stenosis) -as need be.
Further suggestions in this regard would need ore information /investigations after getting more details from You.
