I have RA & have experienced incontinence but I am having extreme lower back pain, and cannot detect urge for bowel movement. I have had accidents, when I wasn t even aware of it! I have been taking rituxan infusions for about 2 1/2 years, but am scheduled to change to actemra because of ankle swelling. I have constant pain & extremely low energy. The bowel problem is new!

Hello and Welcome to ‘Ask A Doctor’ service. I have reviewed your query and here is my advice.

You must be suffering from symptoms of nerve compression probably either due to prolapsed disc or displaced bone due to trauma. The nerves exiting from lower back control your bowel and bladder and also supply back and lower limbs. You need an urgent MRI of lower back (lumbosacral spine) and help of a spine surgeon. You may require operative intervention if there is compression over nerve roots.

Hope I have answered your query. Let me know if I can assist you further.
