Suggest Treatment For Severe Back Pain

My brother who is 66 has been experiencing severe back pain. Tonight we noticed a lemon sized lump on the right side of his spine. The pain is so severe that he cannot walk or stand. He had knee surgery on 10-18, and that seems to be healing well, then all of a sudden, this happened, 4-5 days ago. Because of other heart medication he is one, he cannot take any type of ibuprofen. Please advise
