I have read your query and from your description of the patches on your head and scalp,i can come to two diagnosis provisionally
1st would be
seborrhoeic dermatitis (commonly termed dandruff) and second would be scalp psoriasis(which i think is your case).
Though both can present as lesions in scalp ;a more dry lesion with redness would favour my diagnosis towards scalp psoriasis
scalp psoriasis can come as a single patch or several, and can even affect your entire scalp. It can also spread to your forehead, the back of your neck, or behind your ears.
Please understand it doesnt spread from person to person contact;it is more immunological and try to relax,if you stress over it alongside with scratching it can cause hair loss temporarily
as for treatment i would recommend the following:
-
salicylic acid and coal tar of which are available as otc medications.
- derivatives of vitamin A (tazarotene)and vitamin D(
calcipotriene) are somewhat effective in this condition
-strong steroids for long application
understand all this are to be applied to the scalp and might need 8-10weeks application.
In case you have
severe psoriasis,i would advise you to go for cyclosporine/
methotrexate along with above said vitamin derivatives and steroid.
Use this only as per your doctor's advise and also do a liver function test every two months for the next 8months
Newer class of biologics like
etanercept,
infliximab etc may be taken as last resort and as per your doctors advise.Also ask him whether he would use UV comb if available in clinic in this case.
Since i have no pictures ofyour patches,i have put forward the many modalities we use here.
