General & Family Physician 's  Response
Rash intergluteal cleft is due to repeated infection. Your proneness to infection depends upon your body immune system. There are a number of factors which jeopardize your immune system resulting in increased susceptibility to infection. Those factors include nutritional deficiency, diabetes, HIV infection, compromised skin integrity such as after surgery or trauma, excessive sweating, compromised blood circulation, congenital immunoglobulin deficiency and chronic diseases etc. Intergluteal cleft is a neglected area of the body with regard to cleaniness.
Advice
1.     Apply a cream containing clobetasol, miconazole and gentamicin such as Lobate – GM cream thrice daily
2.     Ibuprofen 600 mg per oral with meal if pain occurs
3.     Fluconazole 150 mg twice weekly per oral for 2 months
4.     Amoxicillin 500 mg thrice daily per oral for 7 days
5.     Investigation – blood sugar (fasting & postprandial)
6.     Wear loose cotton undergarment and clothes. Sweating predisposes infection. You can use dusting powder to keep your buttock dry.
7.     Take high protein diet such as eggs, meat, cheese, vegetable and fruits
Thanks
