Suggest Treatment For Pain In The Right Upper Quadrant Area

Right upper quadrant pain, right under the edge of the ribs. Pressure feels like a baby s foot pushing (no chance of pregnancy). Has been going on since 10/04. Negative US, CT and today normal functioning HIDA scan. I maintain a good balanced diet. Yes, some foods this month have increased the pain and pressure. No diahrrea or anything that would make me think it is intestinal . Being referred to GI. What would they be looking for?? Thanks

Hi.
There are a variety of causes of Rt hypochondriac pain.
Abnormality may lie in organs such as ,the Gall Bladder, Liver, Duodenum, Hepatic flexure of colon, kidney or more superficially , trauma to the muscles and ribs of the chest.
Also, pain on inspiration ( respiratory catch ) may indicate gallbladder and diaphragmatic pleural pathology.

Since USG and HIDA scan has not detected any pathology, CT scan with contrast is needed to look for free fluid, fat stranding , mucosal irregularities, masses of the intestines, Liver, kidneys, adrenals, and the retroperitoneum. Calculi, cysts, anatomic abnormalities will be better detected at CT.

Pelvic organ pathology , uterine/ ovarian in women , rarely transmit pain in the hypochondrium.

Do you also have weight loss? How are the blood tests results? : CBC , CRP, Urine exam and ESR ? Family H/o any disease, previous illnesses need to be correlated.
The treatment shall depend upon accurate diagnosis.
The GI specialists will perform Gastro-colonoscopy to look for , Inflammatory bowel disease, polyps, diverticuli and tumors in the intestines.

Liver biopsy is indicated if you have abnormal Liver function tests.

Rx for now:
Do not overuse pain killers as they have potential adverse effects.
A simple paracetamol should serve the purpose till a diagnosis is reached.
Drink plenty water. May be maintain multivitamins too.
