Suggest Treatment For Pain Due To Piriformis Syndrome

I have been having hip pain and was diagnosed with piriformis syndrome. I spent 6 months total in physical therapy, and my pain is growing. The pain is now in the right half of my back and shooting down my leg. Yesterday my leg was shaking I m guessing from a spasm. What could be going on?

If you have this syndrome. And not relived by physiotherapy. Need to roul out lunber spine pathology. If spine is normal on MRI. Operative release of the pyriformis muscle is next line of management. Local steroid injection can be tried
