Suggest Treatment For High BP And Loss Of Bladder Control

Hi..doc ..good morning.. This regarding my dad who is 77 yrs old ,who is on medication for alzimers for the last 4 yrs . He was stable for first two years , now he is in to moderate stage , he is a heart patient n undergone angioplasty 20 yrs back but his bp levels are stable since then and he is on Aten 25 mg for last 15 yrs and no major complaints ..In these last four years of medication for Alzimers four times he had collapsed suddenly n got hurt and jus recovered , after CT scans we were told that blood flow to his brain is reduced by 30%.now recently his medication is changed by neuropsychiatrist 2weeks back .He is on donetaz 11.5 mg , morning and qutipin 12.5 mg, desponil 12.5 mg, mirtaz 7.5 Mg, depakote xR 250 mg ,,all these at night . After using these medication he has become more restless and disoriented at night and too drowsy at day ..two days back he got lost due to wandering n found him in collapsed state , though he was in his senses. Rushed him to emergency care found his bp levels high at 190/120 at the time of admission .later it came down to 170/90 after an ecosporin injection..At this point my doubt is that is by any chance that his new medication had side effect of raising his bp levels ? He has lost bladder control n peeing at wrong places may be coz of disorientation at nights . Can u plz suggest wht we should do now

1. He is on numerous medications, which act on the central nervous system. Any interaction between these can cause restlessness, disorientation, etc as a adverse effect.
2. Yes, as your suggested, there is no loss of bladder control and it is only disorientation.
3. His medication might require reevaluation as bit of repetition is found in relation to action of 1-2 medications. Also, medications needs to be added slowly to evaluate for adverse events.
4. He will require a full time care taker. Do get one, to take care of medications, personal care, support and nutritional care. Meet your doctor for evaluation of medications.
