Suggest Treatment For Epididymitis In Male

I am a 33 y/o male with epididmyitis. I was treated with a 10 day course of Doxy. There has been no improvement of the pain for the past 3 months. Urine culture was negative. U/S I was told was normal with no signs of cancer. I am in extreme pain and can only walk a short distance. The pain radiates up to the abdomen. I have swelling and tenderness of the testicles. My urologist want to do a cord block next week. Is this a reasonable next step. Or would a longer course of antibiotic be better?

Thanks for posting in HCM.
I understand your concern.
Epididymitis is an inflammation of the epididymis. If it has not got better with the course of antibiotic Doxycycline, it suggests that the infection is still persisting.
Since it has been 3 months after taking antibiotic, before undergoing the procedure as suggested, you can try another course of conservative management.
Kindly do the following as under:
1. Take a course of antibiotic Levofloxacin once a day for 10 days.
2. Take antiinflammatory-analgesic medication Ibuprofen twice a day along with it.
3. Give adequate scrotal support, which is essential for healing process.
4. Improve your intake of fluids to keep yourself well hydrated.
Kindly take the consent from your doctor before you try above measures.
In case the response is not good, you can go for the procedure as suggested by your Urologist.
