Suggest Treatment For Curvature In The Lumbar Spine According To A MRI

I recently had an MRI done and can t get in to see the specialist I was referred to for 3 months! Meanwhile, my imaging center sent my report and I don t understand all of it. or what it means for my future and pain. The following are the report findings: Slight curvature of the lumbar spine convex to the right. Multilevel premature degenerative disc disease, disc dehydration and disc space narrowing, L2-L3 through L5-S1. Vertebral body height is relatively well-maintained throughout. Focal fat in posterior L5 vertebral body. No discrete pars defect. No spondylolisthesis. The bone marrow signal is normal.Spinal cord signal appears normal. The conus medullaris is normal in position. The visualized aorta is normal in caliber. The paraspinal soft tissues appear normal. T12-L1: No disk bulge, spinal canal or neuroforaminal stenosis. L1-2: No disk bulge, spinal canal or neuroforaminal stenosis. L2-3: Small broad-based disc herniation slightly indents the thecal sac. Contributes to mild foraminal stenosis. L3-4: A small broad-based disc herniation more prominent on the right side partially effaces the right corner of the thecal sac. Measures 3 or 4 mm, slightly smaller than previous. Disc herniation and mild overgrowth the posterior elements contribute to mild- moderate canal stenosis. Maximum dimensions of the thecal sac 11 x 6.8 mm. Ligamentum flavum hypertrophy measuring up to 3 mm. Mild narrowing of both neural foramen. L4-5: A moderate-sized disc herniation effaces most of the thecal sac more so on the right side. Thecal sac measures up to 6 x 4 mm. Ligamentum flavum hypertrophy measuring 3 mm. Mild/moderate narrowing of both neural foramen. L5-S1: Small broad-based disc herniation effaces the anterior thecal sac. Contributes to mild- moderate foraminal stenosis and looks similar to previous. IMPRESSION: 1. At L4-L5 a moderate-sized disc herniation effaces most of the thecal sac, contributes to moderate to severe canal stenosis unchanged from previous. 2. Smaller disc herniation at L3-L4 and L5-S1 stable or slightly contracted. (Lumbar MRI)

General Surgeon 's  Response
Hello,

I have gone through your whole MRI report in details and here is my co relation for the given data. First of all the details of symptoms are mandatory to get the necessary aspects of discussion on clinical grounds. The stenosis is of moderate amount and management guidelines depend upon the level of symptoms.

If the symptoms are of mild severity as mild pain, back ache or difficulty in working around assisted physiotherapy, supportive medications, weight control, regular orthopedic follow up for consultation

If the symptoms are of moderate to sever intensity as inability to work to minor extent, no response to conservative therapy or patient demands, surgery of diffusion techniques or else

Hope I have answered your question. Let me know if I can assist you further.

Regards,
Dr. Bhagyesh V. Patel, General Surgeon
