General & Family Physician 's  Response
Hi,
Most common cause of dry, loose productive cough for more than 3 months in chain smokers is acute bronchitis with inflamed bronchioles. This shortness of breath and chest pain over ribs is also due to this continuous coughing.
If you have quit smoking then its good.
You should undergo routine blood with X-ray chest and sputum examination for R/M and AFB to rule out other lung pathology before starting antibiotics.
You will require antibiotics ( quinolones in combination with cephalosporins) with antihistamines or mast cell stabilizers.
Oral or inhalational steroids with nebulizers and bronchodilators on as and when required basis.
Consult pulmonologist for further advise.
