I am a 40year old woman suffering from fatigue and hormone imbalance. I just got my lab test for an ACTH test (blood drawn 8:00 am), my result was 8.9 with a reference range of 10-60. I have low DHEA (3 range of 2-23), low afternoon and evening cortisol (.4) and low estrodial 1.1 (range 1.3-3.3). I have one doctor telling me that my ACTH is fine and another doctor saying that is too low and wants to do further testing for pituitary issues. Is my ACTH low to warrant additional testing.
All your values are on the lower side of normal range. I do not see any requirement of additional testing, unless you are having the following symptoms like weight loss, muscle weakness, fatigue, low BP, low blood sugar (hypoglycemia), abdominal pain, dark patches of skin (occurs in Addison's disease and not in secondary adrenal deficiency due to low ACTH etc.). If you have these symptoms, then further testing primarily with imaging of pituitary may be indicated followed by any other test, if indicated and advised by your doctor.
