I went through your details. First of all you did not mention the age of your child. At childhood, hyperactivity and fidgety is the basis of
ADHD diagnosis. When age progresses, hyperactivity comes down and attention deficit become predominant.
Anxiety is also common in childhood which usually is not associated with ADHD.

consult a psychologist in person. Let the psychologist assess the mental health of your child to determine whether the child has ADHD or not. Most of the ADHDs do not need medicines.
