Suggest Remedies For Toenail Fungal Infection

My right toenail seemed to have gotten a fungus on the nail bed . The toenail fell off and now is trying to grow back ; however, it is thin, has holes in it ,and if I put peroxide on it , it bubbles as if there is an infection. The skin all on that toe is pealing and the nail bed burns at times. I began putting Ketoconazole on it daily with little difference. I am also concerned be cause I just noticed a small black dot on the lower left corner of nail bed . HELP
