Hi,

Noted your history about stenosis in lower back, lifted 20 lbs and got pain with black blotch on the back, also have a history of using heating pad.
The commonest cause I can think about are as follows:
When you lifted the weight the muscles must have gone into spasm causing hemorrhage that is bleeding into the muscles enhanced by heating thus causing black splotch.
I would advise you the following:
Stop any medicines like aspirin that can enhance hematoma formation.
Stop all activities that cause strain on the back.
Take full bed rest.
Anti-inflammatory medicines like Serratiopeptidase.
Painkillers as your Doctor would advise.
Muscle relaxants.
Do not use heating pad at all.
Remember that such injuries take more time.

Hope I have answered your query. Let me know if I can assist you further.

Regards,
Dr. T. Chandrakant, General Surgeon
