Suggest Management Of Blood Sugar In A Diabetic

My father is 76 years old. He had severe urinary infection in May this year. Then he recovered from that. He is also a diabetic. Then he suffered from facial cellulitis in October. That also became quiet severe and finally settled down.
His sugar has been fluctuating a lot during this time frame. Then he got a blood\urine test done last week and the results are:Haemoglobin is 8.0 against the normal range of 13.0 - 17.0,
Serum creatinine is 2.7 against 0.7 - 1.4, Sodium is 128 against 135 - 145 and Total protien is 4.9 against 6 - 8.2.
Am not sure what is wrong? Is his kidney not functioning properly? Or is it because of sugar fluctuations that the other parameters are not ok? Please advise.
