Should I Go For Surgery To Cure Bilateral Varicocele?

Hello doctor, I am suffering from bilateral vericocele. I got married 2.5 years back and still don't have children. My last report says 18 Millon sperm count, far less then the range specified in the report. I consulted a urologist and he advised me a 3 months course ( 1 year back). I believe there is no difference. Do you advice me surgery for the same or there is any other cure for this problem.
