hi, based on your question it seems like the identical twins are having hereditary breast cancer. it is advisable to undergo mastectomy of both breasts, if there was BRCA mutation identified. since you have already undergone mastectomy, reconstruction surgery can be done to create new breasts, this has no worse outcome in terms of survival. if there was some complications during surgery like problem with wound healing or so, you can wait till it is completely healed and then can undergo cosmetic surgery. it is 100% safe. it would not have any impact on your survival. hope I have answered your questions. Thank you
