Is Sudden Elevation In Heart Rate A Symptom Of SVT?

Respected Sir, I am 38 year old male and working as a software engineer in U S A. In the year 2014 on 3rd Nov when i was playing with my child i just jumped to my right once and i suddenly start feeling my heart was beating over 200 times a minute and i start feeling sweating little bit. i sat on the sofa and measuring my pulse. after 3-4 minutes my heart rate came down to around hundred beats. I have visited doctor and he said it was due to anxiety or it could be due to Supra Ventricular Tachycardia. and he put me on Metaprolol 50 mg. i was taking that medication and i asked you a question on this website. and you said not to think more about it. After that i have visited india for a vacation trip and i visited cardiologist. He put me on holter monitor for 24hrs and asked me to check get blood test done for thyroid, Vitamin B12, and Vitamin D. when the result was out he could not find anything abnormal in my holter monitor but he found that i have very less B12 and Vitamin D. Since then i started feeling like there is a sudden jolt in my throat or in my chest. I can t say where it starts exactly but i was feeling that sudden jolt atleast once or twice in a month for a second. after that jolt feeling i was feeling little tiredness for sometime. and then i am back to normal life. But when i consulted my primary care physician in U.S, he asked me whether anyone in my family has heart problem. As my father got heart problem in the past, i have told him about my father. then he asked me to start taking aspirin low dosage (81 mg) daily. So i started taking aspirin since then. Later i have visited India again on 2015 November. I got my full body check up in Hyderabad. after that i went to visit Cardiologist again, he checked my Threadmill test, Eco Cardiogram, and ECG result and said that my heart is perfectly fine and i do not even need to take aspirin. so i stopped taking aspirin too. There after i have visited lot of places and caught cold, caugh and severe throat pain starting from my throat till my right ear when i swallow anything. this continued for a long time. along with this pain, once day on December 27th 2015, suddenly i started feeling my heart beats are skipping. when i consult doctor he said that it is common. But on 28th i started feeling like my heart was beating fast two times and stopped for some time like 2 seconds and started beating for two times again. When this was happening, i was having severe cough when ever i felt there was gap in my pulse. This happened for a long time. So i visited Cardiologist again. At that time he was checking my pulse my heart rate was 142. He asked me to go and get ECG immidiately. When i went for ECG my heart rate was 112. Then the doctor said that i should get the ECG in right time when the pulse rate is high. after that it settled down. Again i started feeling the same symptoms and again visited doctor. I took ECG but my heart rate was 110 during that time. So he put me on Metaprolol 50 mg delayed release and asked me to take 1 in the morning and one in the evening after breakfast and dinner. he asked me to get Holter monitor for 24hrs. One day in the morning i took Metaprolol 50 mg and in the afternoon i got the Holter monitor device to monitor my heart. I did not take this medication in the evening. and the next day morning. so i handed down the Holter monitor device next day. The Cardiologist has identified that there are Supra ventricular Tachycardia symptoms. and asked me to visit Electro Physciologist. I have visited EPS specialist and he said to keep observing for few more days and no need to go for EPS study at that moment. Since then i am taking Metaprolol 50 mg twice daily. But i would like to take second opinion as this issue is bothering a lot in my daily life. I am scared to be with my family and feeling like i may trigger this symptom again any time. So far i did not experience this symptom again. But could you please answer below questions after looking at my test results attached and the description above? 1. May i know if this is symptom of SVT. means do i have SVT? or it was accidentally happened? The last time you said at time it happens in normal people too sometimes. But this time i was having this continuously for 6 days. and this time it was totally different as i felt my heart beat is paused for sometime and started beating again. This happened for long time. 2. Since the doctor asked me to take Metaprolol 50 mg, May i know if this drug can cure the problem or it can only prevents from SVT symptom by controlling heart rate? 3. Would you suggest me to go for an EPS study? 4. How log do i have to be on this drug metaprolol? Do i have to take this medicine for life long?

General Surgeon 's  Response
Hello,

* As per my clinical experience, the sudden elevation in heart rate may not be always sign of SVT, but multiple possibilities can be there as

- stress and anxiety
- low hemoglobin ( anemia )
- dehydration
- fever or other inflammation in the body
- strenuous work or activity
- running or other exercise
- thyroid disturbances ( as hyper functioning )
- valvular issues or heart chamber dilatation
- other systemic causes of organ dysfunction

In my clinic, after primary clinical examination, I do suggest an EKG for baseline evaluation and guidance accordingly.

Hope I have answered your query. Let me know if I can assist you further.

Take care

Regards,
Dr. Bhagyesh V. Patel
