OBGYN, Maternal and Fetal Medicine 's  Response
Hi, I have gone through your question and understand your concerns. The size of the fetal lateral ventricle at 21 weeks should be less than10 mm . Size more than 10 mm is called ventriculomegaly and can be associated with fetal central nervous system anomaly or chromosomal anomaly.
If your wife's ultrasound is shows fetal ventricle to be 8 mm , then it is normal. But the prominence of the ventricle has to be looked for.
I would suggest you to consult a fetal medicine specialist so that a detailed anomaly ultrasound can be done and appropriate decision can be taken.
Hope you found the answer helpful.
Wishing you good health.
Regards,
Dr Deepti Verma
