Hi,I am Dr. Shanthi.E (General & Family Physician). I will be looking into your question and guiding you through the process. Please write your question below.
Hi. My daughter was prescribed Domperidone 30 mg three times a day to help with lactation. It has been sixteen days since starting it in which time my daughter has had little sleep (not the type you miss with a new born) and developed diarrhea. The baby is now nursing well. We asked a pharmacist how to wean off the medication and he recommended cold turkey . What we have read suggests doing it gradually with omitting one pill each day. We haven t discussed the discontinuation with the midwife who ordered it as we have doubts about her experience. What do you recommend?
Hi, Thanks for posting in HCM. Domperidone prescribed for inducing lactation should not be stopped abruptly. The suggested beginning dose is 10 mg 4 times per day for 1 week, then increasing to 20 mg 4 times per day. The increase should be gradual . It is advisable to continue to take the domperidone from the initiation of lactation until weaning. Most mothers complain that when they forget a dose their milk supply decreases. However, once lactation is well established and the baby is breastfeeding well, the mother may consider slowly decreasing domperidone gradually. If her milk supply drastically decreases, she can always increase the dose back to 20 mg 4 times per day. Hope the information provided would be useful.
