Is Discontinuation Of Clexane Advisable When Child Has Blood Clots In Heart?

Hi I m a worried.my baby when she was one month old we discoverd she has very severe sepsis and that she had 3 blood cloths in her heart.they put her on claxene straight away and thanks god the blood cloth disappeared.she is now 6 months old and still on clexane 12mg twice daily.they did the thrombophilia test twice and the results came that the protein s is a bit low i think it is around 41.they did test on us parents to see if it is genetic but we don t have problems.now the doctor told me to stop the claxane in 3 weeks time and I m a bit worried.do I need to worry ? Can she live a normal life with the protein s defiency or she is more prone to form cloths. Do you think she has to take medicine all her life because of the protein s.thanks

Hello,

As per my clinical experience, protein s deficiency is related to mutation in PROS 1 gene of the baby causing hypercoagulable stage and venous thrombosis episodes. It is highly controversial world wide among different schools of thoughts to whether continue the anti coagulant Clexane for rest of life or not.

Usually, if the initial thrombotic episode was life threatening, involved multiple or unusual sites (as mesenteric, cerebral veins) general conclusion of life long anti coagulants. Rest of the cases are recommended for a period of 6 - 9 months intake of Clexane (which is your baby's case) as recurrent episodes are very less likely.

Utmost care in future has to be figured out in the incidence of need of any surgery (particularly ortho related), which requires prior heparin use before the surgery to prevent venous thrombosis episode or event.
