I Have A Terrible Swelling In My Face. It Isn't

I have a terrible swelling in my face. It isn't a "spot", doesn't seem to be an abscess. The swelling is found in the skin that connects my gums to my upper lip, not directly on my gums. It's puffed all across the top of my four front teeth, directly under my nose. The swelling is not inside my nose but it hurts my nose to touch it or wiggle it. I can feel pain in my neck, throat and both my ears feel infected. I've just awakened from a bad sinus headache. I can feel throbbing behind both my ears (pains possibly lymph nodes). This is the third time this has happened this year, is becoming seasonal. The first time this happened, my face swelled huge and pus released itself above my gums and offered immediate relief. ER doctor said sinusitis, prescribed amoxicillin. Second time two months later, I saw my family doc before swelling got out of hand, said could be a tooth issue. Was treated with a z-pack. Now it's back and I'm feeling more pain in my head and neck than before. Swelling just began. I do not feel any pain in my teeth, though they're not in the greatest of shape. Two front teeth have been filled twice. I've been diagnosed with sinus issues since teen years. I don't know what to do or what doc to see. So far answers from docs are slim.
